Your First Appointment

JL Colon Operatories

From your very first visit, our friendly staff will take the time to get know you, your dental history and your health status so we can offer the quality service and specialized treatment that is just right for you.

We'll begin with a comprehensive oral exam and a detailed discussion about your oral history and health. Dr. Colon and Dr. Gaida will spend time learning about your dental habits and concerns and reviewing your previous dental experiences.

Your first visit will finish with education about preventative procedures and recommendations for a treatment plan for optimal dental health and a smile you feel great about.

Before ending your visit, our dental team will help you to understand the cost of any recommended treatments, explain your insurance benefits and schedule your next appointment.

Please bring an updated list af all medications, your insurance card and complete the patient forms or allow enough to complete them upon arrival.

Our Patients Enjoy

  • State-of-the-art procedures, facilities and equipment

Patient Forms

For your first visit, please complete the forms below or arrive 10-15 minutes early to complete them at the office.

Dental Insurance & Payment

Our office accepts many dental insurances. Our experienced staff is here to answer your billing or insurance questions and help you understand your insurance benefits.

Treatment fee quotes will be honored for three months. If you have a participating dental insurance, we will bill your insurance company. However, due to ongoing insurance contract changes, we will only able to approximate your coverage. If your insurance pays less than expected, any remaining balance will be your responsibility. A finance charge may be applied to overdue accounts.

Payment Options

We accept cash, check, Visa and MasterCard.

CareCredit

CareCredit may help you pay for treatments and procedures your insurance doesn't cover. CareCredit offers no interest financing or low minimum monthly payment options. Visit CareCredit.com for more info or to apply.
